Could you add a mandatory check box for an explanation to be provided for a given answer?
There is already a mandatory check box for the questions and also to provide an attachment.

We also have this requirement. There are many usecases where we want to ask the respondent to provide justifications when a specific answer(s) is selected.
For example, we ask a vendor if they perform a certain activity and the options available are Yes, No, Not Applicable.
Currently, we have to create an additional question to force the vendor to give an explanation or an attachment when "No" or "Not Applicable" is selected. This is double work for us when setting up the questionnaire and it also makes the reporting complicated.
It would have been way easier to have an option to force the user to either provide a justification or attachment (or both) if they select "No" or "Not Applicable" is selected.
